Dear Medical Provider:
Thank you for your interest in providing H1N1 Influenza vaccinations in Kentucky. The next step in the process is to enroll your practice with your local health department.
Please find attached eight (8) documents pertinent to this process. The primary document is the "H1N1 Vaccination Packet INSTRUCTIONS" document. Open this document first and follow the instructions to enroll your practice and submit your first order for vaccine. All enrollment forms must be submitted to your local health department - do not return forms to the email address originating this message!
From this point forward, your communications should be directly with your local health department. Contact information for all local health departments in Kentucky has been provided as one of the attachments. Thank you again to all who have agreed to participate in the H1N1 Influenza vaccination effort to protect the citizens of Kentucky!
